Patient Rights and Responsibilities

Understanding your rights and responsibilities as a patient at Hope Wellness

PATIENT RIGHTS

All patients treated by Hope Wellness have the right to:

  • Receive care without discrimination based on age, race, ethnicity, national origin, religion, disability, sex, sexual orientation, gender identity, or source of payment.
  • Be treated with dignity and respect by all staff members, clinicians, and other patients.
  • Receive information about your diagnosis, treatment options, and alternatives in terms you can understand.
  • Participate in decisions about your care and treatment plan.
  • Refuse treatment to the extent permitted by law and be informed of the potential consequences.
  • Privacy and confidentiality regarding your personal and medical information in accordance with HIPAA and other applicable laws.
  • Access your medical records in accordance with Hope Wellness policies and applicable laws.
  • Receive information about Hope Wellness services, providers, and your rights and responsibilities.
  • Voice complaints about the care provided and have those complaints reviewed without fear of retribution.
  • Designate a representative to make healthcare decisions on your behalf if you are unable to do so.
  • Receive emergency care when needed without unnecessary delay.
  • Receive information about charges for services, payment policies, and your financial responsibility.

PATIENT RESPONSIBILITIES

As a patient at Hope Wellness, you have the responsibility to:

  • Be considerate and respectful of other patients, staff, and property.
  • Provide accurate and complete information about your health history, current medications, and any changes in your condition.
  • Actively participate in your care and treatment plan.
  • Ask questions when you do not understand information or instructions.
  • Follow the treatment plan recommended by your healthcare provider or discuss alternatives.
  • Keep appointments or notify Hope Wellness in advance if you need to cancel.
  • Provide accurate insurance information and fulfill financial obligations.
  • Inform your provider about any advance directives or changes to advance directives.
  • Notify staff of any safety concerns or suggestions for improvement.
  • Be cooperative and understanding with staff who are providing care.

PATIENT ACCOMMODATIONS

Hope Wellness provides free aids and services to people with disabilities to communicate effectively with us, such as:

  • Qualified sign language interpreters
  • Written information in other formats (large print, audio, accessible electronic formats, other formats)

Hope Wellness also provides free language services to people whose primary language is not English, such as:

  • Qualified interpreters
  • Information written in other languages
